We appreciate all private referrals. Simply fill in the online form below to send details through to us. Referral information is transmitted to a secure server. All appropriate actions have been taken to ensure the privacy of data submitted online. If you would like to refer a patient to us but are not a Dentist, please use the enquiry form on the Contact us page.
One of our friendly treatment coordinators will be in touch with your private patient to arrange a consultation for them. Alternatively, your patient can call 0151 645 5473 and speak to us directly, or fill in the form themselves.
You might find it handy to add our on-line referral form to the favourites list on your surgery computers. Your staff can fill it out while the patient is in the chair.
If you would like some hard copy referral forms sent to you, let us know. Alternatively, you can download and print a paper version.
Please note that we can only accept NHS referrals via the Cheshire and Merseyside online referral system. Private referrals can be sent directly to the practice via the usual methods.

Most orthodontic treatment is carried out for children aged 10 years and over, and once the permanent dentition is established.
Exceptions to this rule, when referral in the mixed dentition is appropriate, are:
- Anterior or posterior crossbites
- Asymmetry in the pattern of tooth eruption (e.g. a lateral incisor erupts before a central)
- Molars of poor prognosis (seek opinion before extracting)
- Lack of palpable canine bulges buccally (age 10 onwards)
- Supernumerary teeth
- Submerging deciduous molars
- Impacted teeth
Otherwise, please delay referral until the first premolars have erupted.
